Azaria Disability Services

Azaria Disability Service is NDIS provider that encourages community participation, social interaction, increasing mobility, and growing independence.  Having personal knowledge of the needs, hopes and dreams of participants, family and care workers through experience with a close family member, Azaria Disability Services will draw on this insight as well as employ qualified and experienced staff to offer a valued service to the local community.  

Azaria Disability Services’ services include:

• supporting the development of daily living and life skills and the provision of physical wellbeing supports such as physical activity and nutritious diets;
• transport for Participants who cannot use public or other private transport, from their home to their daytime activities, place of study or workplace;
• general household services, such as cleaning, home and yard maintenance, laundry and meal preparation and delivery;
• including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and building confidence and social skills;
• assistance with personal activities and daily tasks, such as personal care, hygiene, dressing, medication and domestic tasks;
• providing time limited support that focuses on addressing barriers and reducing complexity in a person’s support environment, while assisting them to connect with NDIS supports and build their capacity and resilience.
• NDIS Plan Management, assisting NDIS participants to manage their NDIS budgets; 
• sourcing products to enable cooking, cleaning, washing, home maintenance and other tasks;
• enabling participants to access mainstream activities not included under other community participation registration groups; and
• assisting people to strengthen their ability to coordinate and implement their NDIS supports and participate more fully in the community.
