Sydney Health & Care Services is a registered Disability Provider under the National Disability Insurance Scheme and have been providing life-changing services for almost 3 years. With the aim to revolutionise the NDIS, SHCS have been working tirelessly with the assistance of Support Coordinators and Local Area Coordinators to assist participants in reaching their short term and long-term goals.
The services we provide at Sydney Health & Care Services are:

• Participate Community
• Supported Independent living (SIL)
Some of the methods Sydney Health & Care Services use to achieve participant independence and assistance in achieving their goals are:
Initial Consultation
Sydney Health & Care Services conducts an initial face to face consultation at the participant’s home wherein every detail of the service is explained to them all while we collect information on what the participants goals are with his/her NDIS plan. A yearly plan is also drawn up with the participants budget to clearly show how many hours they can comfortably use throughout the year as to maximise the use of their plan.
Compatibility
We work hard in ensuring participants are comfortable when receiving support and to do this we pair participants with a Support worker/Carer who shares the same interest, hobbies, and age similarity. We have found that in using this technique increases confidence and moral in participants and boosts overall progress.
Support worker/Carer ratio.
Sydney Health & Care Services employ a strictly one on one support worker/carer to participant ratio. Unless requested we do not encourage participant group sessions as each participant has individual goals.
Initial Weekly contact
SHCS touches base with participant/participant representative once a week initially to follow up on the compatibility of the participant and support worker and if it is not successful will send in another support worker/carer the following week.
Weekly Reports
Our support workers/carers send through weekly reports which are cross referenced against the participant’s goals to see progress. Sydney Health & Care Services is revolutionising the NDIS with our progressive way of assisting Participants access the full potential of their Plans. The purpose of this letter is to explain the way our company operates and to assist Local Area Coordinators and Support Coordinators in pairing their participants with our company should it suit their participant.
Community Based Programs
Our range of social and recreational programs are focused on empowering people to live life the way they choose. Feeling connected to the community you live in is important for us all as social citizens. Our activities are planned out monthly and sent through our monthly newsletter for our participants to choose from.
Supported Independent Living (SIL)
Supported Independent Living (SIL) is an NDIS support service that involves supervision and assistance with daily everyday tasks to help people with disabilities live as independently as possible. At Sydney Health & Care Services, we offer this service as part of a shared group-home environment, or alternatively, as a drop-in service in your personal homes across Sydney.
The current locations we provide our services to, are:
• Wollongong
• Illawarra
• Shoalhaven
• Sydney
